We collect identifiers (name, address, EIN, email, phone), commercial information (logistics history, billing), geolocation data (active shipment tracking), and usage data (IP addresses, cookies).
We do not sell your data. We share only with MROs and certified labs, government agencies when required for DOT compliance, and logistics partners under strict BAAs.
California residents may request access, deletion, or opt-out of sharing of their personal information. Contact us via the phone numbers on this site.
All data is protected by 256-bit SSL/TLS. Portal data is encrypted at rest and in transit per HIPAA/HITECH standards.
Patient names and diagnoses are never stored on exterior packaging or public-facing tracking logs. PHI is accessible only to authorized personnel.
All chain-of-custody data in our Client Portal is encrypted at rest and in transit using AES-256 encryption.
We execute Business Associate Agreements with all covered entities including hospitals, clinics, and healthcare groups throughout Orange County and California.
In the event of a PHI data breach, all affected covered entities and individuals will be notified within 60 days per the HITECH Breach Notification Rule.
Federal CCFs are used for all DOT collections. Every specimen is documented from collection through laboratory receipt with a complete, unbroken chain of custody record.
All laboratory results are reviewed by a licensed Medical Review Officer prior to release. No DOT result is reported without MRO verification.
Apex Medical OC maintains a Drug-Free Workplace policy per all applicable federal and California state regulations. All collectors and drivers are subject to pre-employment and random drug testing.
